If you forgot your password for "MyDGS" and would like it emailed to you, please fill out the form below.  The password will be sent to the email address on file from your initial registration.

Please note:  If you have not allowed at least 2 business days for your registration to be processed, your password will not yet be on file.  Confirmation is sent once access is granted.

If you have received confirmation indicating your access has been granted yet still cannot log in, make sure you are using your 9-digit employee number ending in 00.  Your password is case sensitive and may not exceed 8 characters.
